Extended Care

Extended Care Service 
Our Extended Care Service provides planned, short-term breaks for families and unpaid caregivers of children and adults  with developmental delays, some behavioral challenges, intellectual disabilities, and adults experiencing cognitive loss. These breaks help support and maintain the primary caregiving relationship while also offering a positive, engaging experience for the individual receiving care. This service  is staffed at an 8:1 participant-to-staff ratio. 

ADULT Extended Care Availability:
  • Fall to Spring (September - May)
    •  Week Days Before & After Adult Day Program: Time is available Monday - Friday from 7:00 - 9:00 AM and/or 3:00 - 5:00 PM on scheduled program days
    • Weekend: 1st and 3rd Saturday of each month from 9:00 AM - 4:00 PM
  • Summer (June - August)
    • Week Days Before & After Summer Camp: Time is available Monday - Friday from 7:00 - 9:00 AM and/or 3:00 - 5:00 PM on scheduled camp days
    • Weekend: Offered on most Saturdays from 9:00 AM - 4:00 PM
YOUTH Extended Care Availability:
  • Fall to Spring (September - May)
    •  Week Day After Adult Day Program: Time is available from 2:30 - 5:00 PM on scheduled program days
    • Weekend: 1st and 3rd Saturday of each month from 9:00 AM - 4:00 PM
  • Summer (June - August)
    • Week Day Before & After Summer Camp: Time is available from 7:00 - 9:00 AM and/or 4:00 - 5:00 PM on scheduled camp days
    • Weekend: Offered on most Saturdays from 9:00 AM - 4:00 PM
General Extended Care Information:
Arrival & Drop-Off:
  • Participants should be dropped off no more than 5 minutes prior to the scheduled start of care.
  •  If a participant will be late, please call 262-947-3660 or write a message in the Brightwheel Communication App to notify staff.
  • If a participant has not arrived within 15 minutes of the scheduled start time, staff will call home.
  • If no contact is made after an additional 15 minutes and no other participants are scheduled, staff may leave the site.
  • Participants should arrive and be picked-up 
Pick-Up:
  • Participants should be picked up no more than 5 minutes after the scheduled end of care.
  • A $15 late fee will be charged for every 15 minutes past the scheduled pickup time.
  • Repeated late pickups may result in ongoing charges or denial of future care.
Meals & Snacks:
  • Meals are not provided. Please bring a lunch
  • Participants may also eat ahead of time at home depending on their arrival time to extended care (lunch is normally eaten around 11:30 am on weekend extended care days)
  • A small snack will be provided
What to Bring and Activity Prep:
  • Participants engage in a wide variety of activities including swimming, ice skating, gym games, fitness activities, outdoor time, and more.
  • Please pack appropriate clothing and swimwear, and label all items.
  • Cold lunch, water bottle, optional an additional snack.
Transportation:
  • Transportation is available on the 1st and 3rd Saturday of the month.
  • Transportation must be scheduled at least one week in advance to guarantee availability.

Medications:
  • All medication must be in a prescription-labeled container with the participant’s name, dosage, timing, and prescribing doctor listed.
  • A current medication form must be on file. Staff only can give medication based on the dosage on the med bottle
  • Medication is only given by trained staff. 
Personal Care:
  • Personal care assistance is provided, but supplies are not included.
  • Please send enough briefs, pads, food, and an extra set of clothes for the day.
  • Swim briefs are required for participants who wear briefs and plan to swim.
Weather / Program Cancellations
Weather Cancellations will be posted to the TR Facebook page no later than 1 hour before program opening. (6:00a weekdays, 8a Saturdays).  A message will also be sent via our Brightwheel App. 

Getting Started
Before joining any of our Therapeutic Recreation (TR) programs or services, an assessment is required. This helps us better understand each participant’s needs, interests, and goals so we can provide the best possible experience.

Step 1: Complete our Interest Form.
Step 2: Once we receive your form, a staff member will contact you to schedule the assessment.
Step 3: After the assessment, we’ll work together to find the right program fit for your child or family member.
